Shillong, Dec 10: Smti Agnes Kharshiing discharged today; from NEIGRIHMS with alleged history of assault on 08.11.2018. On examination she had, Lacerated wound 2.5 cm x 0.5 cm forehead, lacerated wound 2.5 cm x 0.5 cm on frontal region (R), (R) Tempero – parietal lacerated wound 8 x 2 cm, Occipital region lacerated wound 5 x 0.5 cm, (R) Forehead swelling.
Patient’s Blood Pressure was unrecordable and Glasgow coma scale (GCS) was low for which the patient was intubated in Emergency and resuscitated. Scalp wounds were sutured under all aseptic precaution and she was shifted to ICU with ventilator support. All necessary investigations were sent. NCCT head shows non-hemorrhagic contusion over left Tempero – parietal region with (L) Fronto parietal thin subdural haemorrhage.
The Head injury was managed conservatively. Orthopaedics, Cardiology, CTVS, Surgery, Opthal, Physiotherapy consultations were sought. X-ray (R) elbow & forearm showed oblique fracture of (R) olecranon. After ex-tubation, patient was shifted to Neurosurgery ward on 16.11.2018. Initially, she was disoriented to time, place and person with speech disturbances. Patient condition gradually improved and she underwent fixation of right olecranon fracture (CRIF with CCS) under General anaesthesia on 28.11.2018. Post operative period was uneventful. Since the patient’s condition has gradually improved significantly, it was decided to discharge her today with further home medication and medical advice. She is asked to attend follow up in OPD of Neurosurgery, Psychiatry and orthopaedics.
